The Home Plus chart,
built to K.A.R. 26‑42,
on every staff phone.
Home Plus Charting is the resident chart for a Kansas Home Plus adult care home — the face sheet, provider orders, an electronic MAR with controlled counts, the functional capacity screening, negotiated service agreement, and health care service plan as e-signed forms, shift notes, incidents, and quarterly reviews. A Due tab watches every deadline the regulation sets, across the whole home, so a KDADS survey finds the chart already current.

The whole Home Plus chart, on one phone.
From an 8 a.m. med pass to the negotiated service agreement a surveyor asks for — one connected resident record, built for a small Kansas home with a small staff.
Document the pass the way the rule reads.
Every scheduled dose of every active order lands on one med pass board grouped by pass time, with the home’s counts of given, due now, missed, and upcoming at the top. A licensed nurse or certified medication aide taps Document and records given, refused, held, not given, or self-administered — with a reason whenever a dose is not given. PRN doses carry their follow-up, a dose charted more than an hour late is flagged as a late entry, and a correction is a new amendment, never an edit. A given dose of a controlled medication writes its own count row; destructions need two different signers.
- ✓ Facility-wide med pass board by pass time
- ✓ Only nurses & medication aides can document — enforced by the database
- ✓ Verbal & telephone orders received by a nurse, signature due in seven working days
- ✓ Running controlled counts, shift counts with a witness, two-signature destruction
The screening, the agreement, and the plan — signed on the device.
K.A.R. Article 42 asks a Home Plus home to keep three documents current for every resident: the functional capacity screening, the negotiated service agreement, and, when the screening says health care services are needed, a health care service plan with a licensed nurse named on it. Each one opens as a draft prefilled from the last signed version, is signed on screen by staff and by the resident or their legal representative, and is frozen the moment it is signed. The agreement records every service, who provides it, who pays — including KanCare / HCBS — and any service the resident refused with the risks explained and accepted.
- ✓ Versioned drafts — the signed history is never overwritten
- ✓ E-signatures: staff pinned to the login, resident or legal rep drawn on screen
- ✓ Copy-given tracking for the agreement
- ✓ Shift notes tick off the services the current agreement promises
Every deadline in the rule, computed from the chart.
Nobody keeps a tickler file. The Due tab reads the chart each time it loads and lists, across the whole home, everything the regulation puts on a clock: screenings and agreements coming up for annual review (quarterly with a paid nutrition assistant), a health care plan the screening says is needed but is not on file, provider signatures due on verbal orders, quarterly regimen reviews, annual self-administration assessments, open changes of condition, and controlled destructions waiting on a witness. Each item carries its citation and its date, sorted overdue first — and one tap opens the tab where it gets fixed.
- ✓ Overdue, missing, and due-soon — with the K.A.R. citation on every line
- ✓ Change of condition issues a task to the nurses group and closes itself
- ✓ Add-only records: every entry names its author and time, nothing is deleted
- ✓ Whole-chart PDF for a surveyor, a transfer, or the five-year retention file
From admission to survey, one chart.
Home Plus Charting follows the resident’s stay the way K.A.R. Article 42 describes it — and keeps the proof as it goes.
Screen & admit
Add the resident, complete the functional capacity screening before admission, and fill the face sheet — code status, allergies, physician. If the screening indicates health care services, a nurse is named on the plan.
Agree & plan
Build the negotiated service agreement — each service, who provides it, who pays, anything refused with the risks accepted — and sign it on the device with the resident or their legal representative. Record the copy given.
Chart every shift
Nurses and medication aides document the pass on the board, counts update themselves, staff write shift and service notes against the agreement, and any incident is logged with the action taken.
Stay current
The Due tab surfaces reviews, re-screenings, signatures, and changes of condition before they lapse. When a surveyor arrives, print the whole chart to PDF — it was ready all along.
The Home Plus resident record, end to end.
Face sheet, orders, eMAR, controlled counts, the three Kansas forms, notes, incidents, reviews, change of condition, the Due tab, and a whole-chart PDF — in one login with the rest of MyLTC Apps.
Where each K.A.R. Article 42 requirement lives.
Home Plus is Kansas’s small-home adult care license: a residential setting, a handful of residents who need more care than they can get at home, licensed and surveyed by KDADS, and often funded through KanCare and HCBS waiver services. The chart maps to the article section by section.
Home Plus Charting is part of the Kansas long-term care software MyLTC Apps already runs for skilled nursing facilities, assisted living, and adult care homes across the state — the same login that carries My Tasks, the compliance suite, Meetings, policies, and Alerts. A Home Plus home turns on the chart and the operational modules it needs, on the phones staff already carry, with no server to install. We are built in Alva, Oklahoma, just across the line, and we answer the phone at 620-213-1377.
A clinical record, held to a clinical standard.
Home Plus Charting is a resident’s clinical chart, so it is built to the same HIPAA standard as the rest of MyLTC Apps and then some — encrypted, access-controlled, and hosted in U.S. data centers. Who can chart is decided by group membership in the database, not a checkbox: only a licensed nurse or medication aide can document a pass, only a nurse can take a verbal order, and the shared demo login can read a chart but never write one. Every MAR entry, note, count, and signature is pinned to the real staff member who made it, records are add-only, and nothing is ever deleted — the audit trail the regulation describes is the only way the software can work.
Which plan fits a Home Plus home?
One flat monthly price per facility. Every plan includes the whole core platform with unlimited staff logins; you pick which paid modules fill its slots and can swap them anytime. Here is what a Home Plus home typically turns on.
- ✓ Compliance — the K.A.R. task libraries and survey-readiness dashboards beside the chart

- ✓ Shift Scheduling — the Day / Evening / Night rotation a small staff runs, with swaps and open shifts
- ✓ Alerts & SMS — reach every caregiver at once — a change of condition, a callout, a weather alert
- ✓ Dashboards — the operator’s full-page view of what is due, open, and overdue

Kansas Home Plus charting FAQ
How the eMAR, the three Kansas forms, the Due tab, and the whole-chart PDF keep a Home Plus home’s record current and survey-ready.
What is Kansas Home Plus (Home+) charting software?
A Kansas Home Plus home is a small, residential adult care home licensed and surveyed by the Kansas Department for Aging and Disability Services (KDADS) under K.A.R. Article 42, caring for a handful of residents who need more help than they can get at home — many of them supported through KanCare and HCBS waiver services. Home Plus Charting in MyLTC Apps is the resident chart built to that article: a face sheet, provider orders, an electronic medication administration record, controlled-substance counts, the functional capacity screening, negotiated service agreement, and health care service plan as e-signed forms, shift and service notes, an incident log, quarterly regimen reviews, and a Due tab that tracks every deadline the regulation sets. It runs on the phone or tablet staff already carry.
Which Kansas forms does Home Plus Charting include?
The three forms K.A.R. Article 42 asks a Home Plus home to keep for every resident: the functional capacity screening (K.A.R. 26-42-201), the negotiated service agreement (K.A.R. 26-42-202), and the health care service plan (K.A.R. 26-42-204). Each is a versioned draft prefilled from the last signed version, signed on the device by staff and by the resident or their legal representative, and frozen when it is signed. The agreement records each service, who provides it, who pays for it — including KanCare / HCBS — and any service the resident refused along with the risks explained and accepted. The screening’s field set follows the elements of the department-specified form and should be checked against the current KDADS form before a home relies on it.
How does the medication administration record (MAR) work for a Home Plus home?
Every scheduled dose of every active order appears on a facility-wide med pass board grouped by pass time, and a licensed nurse or certified medication aide documents it as given, refused, held, not given, or self-administered — with a reason whenever a dose is not given. Only those two roles can write a MAR entry, aides cannot document an injection, PRN doses carry a follow-up, a dose charted more than an hour after it was given is flagged as a late entry, and a correction is recorded as an amendment rather than an edit. Controlled medications keep a running count with shift counts, receipts, and a two-signature destruction log.
How does it keep a Home Plus home ready for a KDADS survey?
Everything the regulation puts on a clock is computed from the chart itself and listed on the Due tab, across the whole home: screenings and agreements coming up for annual review (quarterly when a paid nutrition assistant is involved), a health care plan that the screening says is needed but is not on file, provider signatures due within seven working days of a verbal order, quarterly medication regimen reviews, annual self-administration assessments, open changes of condition, and destructions waiting on a witness. Each item carries its K.A.R. citation and a due date. Records are add-only — every entry names its author and time, nothing is deleted, and a whole-chart PDF prints on demand for a surveyor or the five-year retention file.
Does Home Plus Charting replace our clinical EHR, and is it available now?
For most Home Plus homes there is no clinical EHR to replace — the chart has lived in a binder. Home Plus Charting is that binder, built to K.A.R. Article 42 and running inside MyLTC Apps alongside tasks, meetings, policies, alerts, and compliance for the same facility. It does not interface with a pharmacy (orders are entered from the label) and it does not file KanCare HCBS documentation. It is live in the MyLTC Apps demo today; request a demo and we will walk through a seeded chart with you and set up your home.
